Personal health information collected in the course of providing psychotherapy services, including session notes, treatment records, intake forms, and any information you share with me directly
Information submitted through the contact form on this website is used only to respond to your inquiry and is not stored beyond what is necessary for that purpose.
How I Use Your Information
Your personal health information is collected and used for the purpose of providing psychotherapy services, including:
Communicating with you before, during, and after our therapeutic work together
Maintaining accurate and complete clinical records
Fulfilling my legal and professional obligations as a Registered Psychotherapist regulated by the College of Registered Psychotherapists of Ontario (CRPO)
I will not use or disclose your personal health information for any other purpose without your expressed written consent, except where required or permitted by law.
When I May Disclose Your Information
With only limited exceptions, your personal health information will not be shared with anyone outside of our therapeutic relationship without your knowledge and written consent. Exceptions include:
Where disclosure is required by law, such as situations involving risk of harm to yourself or others
Where the CRPO requests access to records as part of a regulatory audit or complaint process
Where disclosure is otherwise permitted under PHIPA
How I Store and Protect Your Information
Your clinical records are stored securely through Jane App (janeapp.com), a Canadian, fully encrypted electronic health records platform that complies with PHIPA and Canadian privacy legislation. Electronic communications are conducted through a secure, password-protected work email.
Paper records, if any, are kept in a locked and secure location. All reasonable precautions are taken to protect your information from unauthorized access, use, or disclosure.
How Long I Retain Your Records
In accordance with CRPO standards and applicable legislation, client records are retained for a minimum of 10 years following the last date of service.
Your Rights
You have the right to:
Request access to your personal health information
Request corrections to your personal health information
Withdraw consent for the use or disclosure of your information, subject to legal and professional limitations
Make a complaint if you believe your privacy rights have been violated
